Comparative Evaluation of Autologous T-PRF Versus L-PRF Along with Freeze-dried Bone Allograft in the Management of Intra-bony Osseous Defect—A Report of Two Cases

Abstract

Periodontitis is a multifactorial inflammatory disease, characterized by formation of osseous deformities. Periodontal bone loss that presents with a vertical defect, is also known as an intra-bony defect. Different types of bone grafts may be employed to restore the compromised attachment structure. Freeze-dried bone allograft is a type of allograft which is osteoconductive in nature and acts as a scaffold. Platelet concentrates are biological products, filled with growth factors leading to periodontal regeneration such as leucocyte platelet-rich fibrin and titanium-prepared platelet-rich fibrin. The aim of the present case report was to evaluate and compare reduction in probing pocket depth, gain in clinical attachment level and gain in bone fill in intra-bony defects using freeze-dried bone allograft along with titanium-prepared platelet-rich fibrin versus freeze-dried bone allograft along with leucocyte platelet-rich fibrin. Two patients, each presenting with an intra-bony defect, were chosen and underwent one of the two methods. The patients were then recalled after three and six months for clinical and radiographical evaluation. Results from the present case report proved that both biologic autologous platelet concentrate membranes were found to be effective in reducing the probing pocket depth and gain in bone fill radiographically. Freeze-dried bone allograft with titanium-prepared platelet-rich fibrin gives marginally more bone fill as compared to freeze-dried bone allograft with leucocyte platelet-rich fibrin
