Abstract

AbstractHazard reporting form the basis of the safety culture of any organization. Different industrieshave different types of techniques which they have built for themselves for the reporting ofhazards. The major theme on which these systems work is behavior based safety. Thesesystems involve the active participation of all the employees who are trained to report anykind of hazards that they see in their surrounding workplace so that they can be resolves atthe earliest. But there are some bottlenecks which are present whenever a system is beingdesigned which affect the effective and efficient performing of this process. Here in this paper, authors have tried to categorize all those possible reasons which can affect a hazardreporting system (by human) with the help of a survey conducted in an Indian based Oil andGas industry and respective measures to overcome such bottlenecks have also been discussed. Keywords: Hazard reporting, failure, analysis, survey, bottlenecksCite this ArticleMadhurima Ganguly, Rahul Aynyas,Abhishek Nandan et al.
