Abstract

Flexible and wearable radio-frequency (RF) antennas are important enabling components for body-centric wireless systems because they provide the communication interface between physiological sensors, wearable nodes, personal gateways, and remote healthcare platforms. Unlike conventional antennas mounted on rigid circuit boards, wearable antennas must operate in close proximity to lossy, dispersive, heterogeneous biological tissues while tolerating bending, stretching, compression, perspiration, laundering, and repeated attachment. These coupled electromagnetic, mechanical, environmental, and human-factors requirements make their design substantially more complex than that of a conventional antenna. This review examines the materials, antenna topologies, fabrication methods, body-interaction mechanisms, wireless body-area network (WBAN) architectures, healthcare applications, safety evaluation, and measurement practices relevant to flexible and wearable RF antennas. Textile substrates, polymer films, elastomers, conductive fabrics, metallic yarns, printed inks, and hybrid conductors are compared in terms of dielectric stability, conductivity, conformability, durability, and manufacturing scalability. Planar monopoles, dipoles, patch antennas, slots, loop structures, and multiband designs are discussed alongside techniques such as artificial magnetic conductors, electromagnetic bandgap structures, and metasurfaces for controlling backward radiation and body loading. The review also considers the effects of tissue proximity, air gaps, antenna placement, user motion, and material moisture on impedance matching, radiation efficiency, gain, and exposure metrics. Healthcare use cases include continuous vital-sign monitoring, activity and posture tracking, patient telemetry, rehabilitation, emergency response, and connected garments. Particular attention is given to the distinction between an antenna that transports data and a sensor or diagnostic algorithm that generates clinically meaningful information. A practical validation workflow is proposed, covering material characterization, on-body simulation, phantom measurements, bending and washing tests, RF performance, and exposure assessment. Key research needs include standardized reporting, long-term reliability, reproducible human-body testing, energy- efficient integration, privacy-aware system design, and clinically meaningful validation. The review concludes that wearable antennas are promising for connected healthcare, but translation beyond laboratory prototypes requires co- design of the antenna, garment, sensor electronics, communication protocol, user experience, and safety evidence.
