Total Laboratory Automation in Resource-Constrained Settings: Cost-Effectiveness, Workflow Transformation, and Quality Outcomes
Abstract
Total laboratory automation (TLA) has emerged as a transformative approach to enhance efficiency, quality, and safety in clinical laboratories. By integrating automated pre-analytical, analytical, and post-analytical processes, TLA has the potential to reduce errors, shorten turnaround times, and optimize resource utilization. However, the adoption of TLA in resource- constrained settings remains limited due to high capital investment requirements, infrastructural challenges, and workforce adaptation barriers. This review examines evidence on the cost- effectiveness of TLA, its impact on workflow transformation, and the resulting quality outcomes in low- and middle-income countries. We also explore strategies for contextual implementation, highlighting approaches that maximize clinical and operational benefits while mitigating financial and technical constraints. The practical difficulties of incorporating automated systems into current laboratory workflows such as equipment compatibility, employee training, maintenance needs, and the accessibility of technical support are given special consideration. The evaluation also considers how sustainable automation may be supported in labs with limited infrastructure through phased adoption, suitable technology selection, and effective resource allocation. This study outlines critical factors for attaining dependable and economical laboratory automation by looking at both the possible advantages and implementation challenges. Insights from this review provide guidance for policymakers, laboratory managers, and healthcare stakeholders aiming to optimize laboratory services in constrained environments while enhancing long-term sustainability, operational effectiveness, and service quality.
